**Qualifying Life Events: Changes to coverage due to a QLE (birth of a child, loss of other coverage, marriage, divorce, etc.) must be completed within 30 days of the QLE. You must also submit documentation of the QLE within that 30 days before approval of any changes can be made. Examples of proof are copy of new insurance card, birth certificate, marriage certificate, adoption papers, etc. (More information is available on the SHP website @ www.shpnc.org). Documentation of the QLE should be submitted to Kelly Vaughn, Health Benefits Representative, at email@example.com.
