Application for Approved Absences: This form must be completed and submitted to the principal at least one week prior to the student’s planned absence(s) for consideration of excused absences. Students may only receive up to 3 days of excused absences throughout the school year. In order for absences to be approved, students must be involved in educational activities each day of the requested approved absence. Please collaborate with your child’s teacher to complete this form.
