This is an important message for the parents and guardians of our kindergarten students, as well as students in grades 1-12 attending school in North Carolina for the first time.
In accordance with North Carolina law, all newly enrolled kindergarten students must have a completed health assessment (physical) and up-to-date immunization records on file at school by the 30th day after enrollment. For this school year, the deadline is September 23, 2025.
The health assessment form was included in your child’s kindergarten enrollment packet and must be completed by a licensed health care provider. If your child has had a wellness visit within the past 12 months of the first day of school, that visit can be documented on the form and accepted.
If your child has not yet completed these requirements, please schedule an appointment as soon as possible with your child’s healthcare provider or the Rockingham County Health Department. For questions or more information, please contact our school nurse.
