This is a very important message for parents/guardians of our kindergartners.
Per NC statutes, all newly enrolled kindergarten students are required to have a health assessment (physical) and a completed immunization record on file at school by the 30th day after enrollment. The deadline for this information is 8/14/24. The blank health assessment form was supplied within the kindergarten enrollment packet and must be completed by a licensed health care provider.
A wellness visit within the past 12 months of the first day of school can be accepted and documented on the provided form. If your child has not already received these health requirements, please schedule an appointment now by contacting your child’s health care provider or the Rockingham County Health Department. For questions or more information, please contact our school Nurse Ellis at (336) 548-2472 or sellis@rock.k12.nc.us.
